Appointment
The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
Patient Forms
If you are new to our office, please visit our patient forms page and download our new patient bundle. These forms will need to be filled out when you arrive at our office. Printing them, filling them out, and bringing them with you will allow us to attend to your medical needs more quickly. However, you may still choose to complete them upon your arrival. Please contact our office with any questions you may have.
Payment Options
We accept all major credit cards, flexible spending accounts, health savings account cards, and CareCredit.
We also take personal checks and, of course, cash.
We offer a 5% discount for those who pay in full for the complete treatment plan at least 48 hours in advance of their treatment. To receive this discount, payment must be made using cash or check.
If you are 67 years of age or older, we also offer a senior discount of 10%. Please mention the discount so we can apply it to your account.
Even if you have insurance, we collect one-third of the payment for all work over $400 at time of service.
For most patients with insurance, we will bill your insurance first. After we receive a payment from your insurance company, we will then collect the remaining amount from you.
Our least favorite part: if we do not receive any payment from you for over 90 days, we will send a warning letter with instructions on how to avoid collections. If you do not respond to the letter within 30 days of the date of the letter, we will assume you are avoiding us and will send you to collections.
Collections can severely impact your credit and cause many problems for you in the future. Please work with our office, and allow us to help you avoid collections at all costs.
Dental Insurance
Our office welcomes all patients with or without insurance.
Dental insurance can be confusing and is meant to be a financial aid that helps cover the cost of dental treatment. Rarely does dental insurance act as a pay-all. Every insurance plan is different, and we cannot know all the specifics of a particular insurance plan. At our office, all patients, regardless of insurance coverage, will pay the full amount due at the time of their first appointment. Patients with insurance will be reimbursed for any over-payment after their insurance company has paid their portion. Our office will submit all insurance claims for those patients with insurance. Any balance due will be billed directly to the patient.

Delta Dental Premier
All other plans are considered out of network.
We do not accept HMO. They will not make payments for work done at our office. Out-of-network PPO plans will make payments for work done at our office.
